Chapter 4.32 PROHIBITED CONDUCT IN LIQUOR ESTABLISHMENTS
4.32.030 Application for permit.
Any person desiring such identification permit shall file written
application therefor with the city clerk, giving his or her name, address and
previous place of employment. They shall then be photographed and fingerprinted
after which the identification application shall be submitted to the chief of
police of the city for investigation and approval. In the event the application
is approved, the permit shall be issued, on such forms, interim and permanent as
are approved by such chief of police. Each such permit shall be valid for a
period of one year, after which a new application shall be filed by any person
desiring a current identification permit. A charge of fifteen dollars ($15.00)
shall be made for the filing of such application or the issuance of the
identification permit. Before any permit shall be issued as provided for in this
article, the applicant shall furnish to the city clerk of the city the written
statement of a medical doctor, based upon an examination made within ten days
thereof, and showing that the applicant does not have any communicable disease.
Such examination shall include a serology examination and gonorrhea smear.
