Chapter 5.28 VEHICLES FOR HIRE
5.28.030 License--Application--Requirements.
Each person desiring to drive a vehicle carrying passengers for hire
within the city shall make application to the city clerk on a form prescribed
and furnished by the city which shall show the name, social security number,
place of residence, length of time applicant has lived in the city just
preceding date of application, date and place of birth of applicant, names of
two citizens of the city who will recommend applicant as being of good
character, evidence that the applicant possesses the necessary driver's license
required by current state or federal law for the type of vehicle the applicant
will be driving, the number of the license and date of issuance, a statement
that applicant is willing to be photographed and fingerprinted by the police
department, a statement that applicant will submit to a written examination on
the driving laws of the state as well as any item concerning the operation of
vehicles for hire as provided by this code; a statement that such applicant will
submit to a driver's test if required by the police department, and a statement
that applicant has not within two years preceding date of application been
convicted of a felony or of any violation of the intoxicating or alcoholic
liquor laws of the state or ordinances of the city. The applicant shall further
state that the applicant is not suffering from any disease or physical
disability that would impair the applicant's ability to safely operate the type
