The city clerk shall keep a record of the permits issued under the provisions of this chapter, showing the date of issue, to whom issued, the name, age, sex and ethnicity of the deceased, of what disease or ailment the death occurred, and the number of the lot, block and addition. Prior to issuance of any such permit, the city clerk shall charge and collect therefor, for the benefit of the cemetery fund of the city, as determined by the city commission from time to time by resolution.
